Licensing requirements for Oregon healthcare and pharmaceutical companies and individuals.

Application processing time is approximately 4-6 weeks, but could be longer depending on the circumstances. Be sure to submit a complete application to avoid delays in registration.

All fees are non-refundable
All applications submitted to the Board that are not complete and processed within 6 months from applicant signature will be expired. Once expired, applicants who wish to continue with the application process must reapply by submitting a new application, along with all documentation, and all fees.
Each company or location address, even if under common ownership, must submit a separate application for registration
The Board can only accept payment by check or money order.
Applications indicating the following activities may require additional paperwork: Consulting, Remote Processing, Central Fill, 503B Outsourcing Facility
The application is to be signed by an owner, partner, etc.
Before you Apply:
Oregon Controlled Substance Registration. The Controlled Substance Registration is required for all outlets that dispense controlled substances. Be advised that the Controlled Substance Registration is not an independent registration. It must be issued in conjunction with a Drug Outlet Registration.
License/Registration Verification in Resident State (required only for applicants located outside of Oregon) Applications for out-of-state pharmacies will not be processed without this verification.
Every non-resident pharmacy must have a pharmacistin-charge (PIC) who is licensed in Oregon within four months of initial licensure of the pharmacy.
An Oregon licensed PIC must be normally present in the pharmacy for a minimum of 20 hours per week.
Provide the following information for all owners, corporate officers, members/managers, and executives. For corporations, list the names of the stockholders who own the five largest interersts

Business name and owners listed on this application must match home state verification
Copy of most recent inspection report (required only for applicants located outside of Oregon).If this facility performs sterile compounding, the sterile compounding inspection report is also required.
If you answer “YES” to any disciplinary action questions, including pending disciplinary actions, all notices, citations, etc. and fully executed Board orders must be provided along with a detailed explanation.
Legible 8.5” x 11” floor plan which identifies the location of sinks, refrigerators, windows and doors. Windows and doors must be marked as secured or unsecured.The minimum equipment requirement to open and operate a retail drug outlet and institutional drug outlet in the state of Oregon shall consist of not less than the following:
(1) The most current issue of at least one pharmaceutical reference with current, properly filed supplements and updates appropriate to and based on the standards of practice for the setting.
(2) Current and properly filed Oregon Revised Statutes, Chapters 689, and 475; current and properly filed Oregon Administrative Rules, chapter 855; and a minimum of three years of the Board of Pharmacy quarterly newsletters maintained in house or other readily retrievable means.
(3) Official Poison and Exempt Narcotic Register if poisons and exempt narcotics are sold or distributed.
(4) Suitable refrigeration.
(5) A sink with running hot and cold water.
(6) Equipment and supplies appropriate to and based on the standards of practice for the setting as determined by the Pharmacy and Pharmacist-in-Charge.
(7) Failure to have and use equipment necessary to your practice setting constitutes unprofessional conduct for purposes of ORS 689.405(1)(a).
(8) If an outlet files original prescriptions electronically, then the outlet must have a computer and software capable of storing and accessing electronically filed original prescriptions.
(9) A pharmacy that dispenses prescriptions for a patient's self-administration must post signage to provide notification of the right to free, competent oral interpretation and translation services for patients who are of limited English proficiency, in compliance with federal and state regulations.

Controlled Substance Registration - In addition to a pharmacy license, controlled substance registration registration is required in many states for pharmacies that distribute controlled substances.
NABP (National Association of Boards of Pharmacy) - Host a variety of programs and resources relating to pharmacist and pharmacy licensure and examination.
Pharmacist in Charge - A licensed pharmacist designated by a pharmacy to act as the party responsible for compliance with regulations.
VPP (Verified Pharmacy Program) - A program run by NABP that allows state boards to share information and more easily register out-of-state pharmacies.
